10-Minute Pre-meeting

  • Each member of the DCT should have reviewed any relevant information about the student prior to this meeting including their SAGE notes and referrals, DegreeWorks, unofficial transcript, unit-specific records, etc.
  • The case manager will then talk about the direction of the meeting and an overall plan based on the collective information shared.
  • Consider the best seating arrangement to ensure the student feels comfortable.

30-Minute Meeting with the Student

  • The SCO case manager will facilitate the meeting.
  • The Case Manager “frames” the meeting by:
    • Starting introductions around the room
    • Explaining the purpose, expected outcomes, and the role of the student in the meeting. Affirm that the meeting is not meant to be punitive, but rather an opportunity for the student to share their experience and to have a network of support help them reach their goals.
  • The Case Manager begins by asking the student what their experience on ___ (academic standing) has been like. This can start a conversation about where they are now and where they want to be, then begin planning toward that goal.
  • All team members should feel free to jump in at any time and share tips and helpful resources from their perspective.
  • Each member should be prepared to ask questions related to their area:
    • Academic Advisor: Academic Plan for how the student can move forward in their current or desired degree.
    • Academic Coach: Study skills, academic challenges, time management, and connection to academic resources (tutoring, presentation support)
    • Case Manager: team of support moving forward
  • The Academic Advisor should ensure the student clearly understands their academic standing, and any requirements of their academic standing.
  • Consider the student learning outcomes:
    • Identify and articulate their educational goals
    • Reflect upon internal and external factors that impact their academic performance
    • Identify and articulate strategies for personal and academic success
    • Apply these strategies to reach their educational goals
    • Evaluate strategies and adjust as needed
  • Name concrete next steps—what does the student need to do in the next 24 hours, next few weeks, later this semester, etc.?
    • How will this help them reach their educational goal(s)?
    • What resources are available to help them reach that goal?
  • The case manager will bring the meeting to a close and be in charge of watching the time.

5–10-Minute Post-DCT Meeting

  • While still in the room, debrief and the SCO case manager will create a success plan in SAGE. The DCT members discuss any additional follow-up needed and who is best to follow up.
  • The SCO case manager sends a copy of the success plan to the student as a follow-up email. Make any necessary referrals in SAGE.

The Academic Advisor ensures that a DCT Meeting type is recorded in SAGE and notifies the school/college representative who will remove the registration hold.
